Liability Waiver and Parental Consent

To be read and signed before participation. Any minor participant (under 18) must be registered by a parent or legal guardian.
1. Acknowledgement of Risk
Basketball is a contact sport with inherent risks of injury, including but not limited to: sprains, fractures, concussions and other musculoskeletal injuries. By registering my child, I acknowledge that I understand and accept these risks.
2. Health Declaration
I confirm that my child is in good physical health and fit to participate in high-intensity basketball activity. Any medical condition, allergy or medication has been disclosed to Basketball 100 Limites at registration.
3. Emergency Medical Authorization
In case of emergency and if I cannot be reached, I authorize Basketball 100 Limites and its staff to take any reasonable measure, including transport to a medical facility, to ensure my child's safety.
4. Release of Liability
I release Basketball 100 Limites, Maxime Louis-Jean, its coaches, volunteers and representatives from all liability for injury, loss or damage suffered by my child during camps, training sessions or activities, except in cases of proven gross negligence.
5. Image Rights
I authorize Basketball 100 Limites to use photos and videos of my child taken during activities for promotional purposes (website, social media, marketing material). I may withdraw this consent in writing at any time.
6. Cancellation & Refund Policy
Cancellation 14 days or more before camp start: full refund. Between 7 and 13 days: 50% refund. Less than 7 days: no refund, but transfer to a future camp possible subject to availability.
7. Code of Conduct
My child agrees to respect coaches, other players, equipment and facilities. Any disrespectful, violent or dangerous behavior may result in dismissal without refund.
8. Payment
Payment by Interac e-Transfer only. My child's spot is confirmed only after payment is received.
9. Governing Law
This document is governed by the laws of the Province of Quebec, Canada.
Signature
Online registration via the Basketball 100 Limites website constitutes electronic acceptance of this document. A signed copy can be requested at any time at basket100limites@gmail.com.
